I know I need a standing of 25 to have Cosa Nostra trust me but once they do what does they special skill they teach cost in skill points?

Yes you have to have the Faction points for them to teach you, then you have to spend the Skill points to learn it.

I think what she meant Ran was "how much does the skill cost in skill points?"

According to our Help file...

Pugilism (Green Hand) - 14 skill points to learn [Toughness]
Safe Cracking (Cosa Nostra) - 12 skill points to learn [Smarts]
Surveillance (Circle of Light) - 15 skill points to learn [Smarts]
Pick Pocketing (Dies Arcanum Brotherhood) - 12 skill points to learn [Toughness]
Hypnotism (Order of Socrates) - 12 skill points to learn [Charming]
Underworld Connections (Eastern Triads) - 17 skill points to learn [Charming]

If you are Very Tough, all Tough skills will be reduced in price by 4 points.
If you are Very Smart, all Smart skills will be reduced in price by 4 skills. If you are Very Charming, all Charming skills will be reduced in price by 4 points.
If you are Tough and Smart, all Tough and Smart skills will be reduced by 2 points.
If you are Tough and Charming, all Tough and Charming skills will be reduced by 2 points.
If you are Smart and Charming, all Smart and Charming skills will be reduced by 2 points.

I'm pretty sure this is all accurate.
