Do the twists make any difference?

Of course the twists which affect factions make a difference, but how about when you dream about the photos, or when the reporter is kidnapped? Does that influence the gameplay in anyway, or not? Whenever I solve a crime, and the twist is about the old lady in the subway, at the end it says that the guy was an international bad-guy, and thanks to me they'd never catch him, so I tried quitting the next case this twist showed up, but I didn't get anything for it. It just tainted my record! Bummer!

The twist that has the most impact is the one involving factions. So I don't quit a case based on the twist unless I'm trying to get in positive with that faction. The other ones are more to make it interesting, and they never thank you if you quit the case and they were able to catch the guy.

Other twists seem to happen on a random basis to give story to the cases, like the random introduction that appears at the start.

But they don't really change anything, right? I mean, if you get the twist where someone hands you a note with your name written on blood, there's no chance that you'll get killed.

One thing I noticed is that when you get the twist where someone says it'd be a pity if someone messed up your case, the witness who suspects the actual killer always gets killed (i.e. if you visit her after the twist). Then you can only get the killer with physical evidence.

Correct, there is no change that you'll get killed.

Your observation about only being able to get the killer with physical evidence after a suspect dies is incorrect. There will always be two pieces of WE pointing to the killer, so at least one witness will still be alive. I'm not sure if it is even true that the killer targets one of the witnesses against them, I always thought it was random who died.
