I just completed my VH in London, and then immediately completed a favour for the Concierge, after which I solved three cases, and then asked the townies as usual but they have no favours!

I remember that I had exactly three cases left and I'd stored two in the case files after searching the crime scene, solved one, then reopened those two and solved them as well.
(I remember clearly because my garage allows only two case files; garages are rather cheap to move around in VHs.)
Anyway, I'm now shifting my HQ to Cairo.

Favours aren't actually worked out on a strictly solve four cases (including the original favour) and get a second one basis. In your case you were probably due a second favour after solving your first favour, only as it was a favour you solved you didn't get the second favour. Consequently you'd need to solve four cases to get a favour again.
