Do all the "twists" have any real effect on solving the case? I mean...I know that if one faction has an interest, you get the faction points, but it doesn't make the case any easier or harder to solve. If your (detective's) life is threatened, can you actually be killed or otherwise incapacitated?

No not all twists have a real affect. The only ones that do is the one you mentioned, and the one with Larry the toe who knows the location of Lucy's cabaret.
Lucy's cabaret is a waste of cash. Unless you want an expensive charming dress.

A threat is just a threat, nothing happens.

So your standings with shady won't go down (so he will even rip us of a bit more)?

No, the standings with shady never change - he just gets greedier as we gain experience points.
If the standings with shady got worse, I'm not sure we'd notice anyway, considering how expensive shady already is. :)

Just realized how what I wrote can be read - it is definately NOT a plea to make shady more expensive.
